Special 3rd Registration Opportunity |

A special opportunity to register in person for the 2009/2010 Fall and Spring season will occur on Tuesday June 23, 2009.
The registration will be held from 7 - 8:30 PM at the Grayslake Park District building at
240 Commerce Drive

2009-2010 Season Registration |

Registration is open for the 2009 fall/spring season.
You'll need to
- Print TWO (2) copies of the player form for each player. Be sure to include your player’s uniform size on the form.
- We are in need of Referees and Coaches. If you would be able to volunteer for one of these positions, please download TWO (2) copies of the volunteer form and indicate one or both positions. Without enough coaches and referees, we will be forced to start a waiting list!
- Print and sign the Parent Sign Off form.
- Print and sign the Kidzone Pledge form.
- Write a check for the fees listed below.
- Send all the the forms along with your check to

No registration will be accepted without all the forms and fees submitted. If your child is 4.5 to 6 years of age, please read the guidelines for this division and submit accordingly.
Fees: $115/first child, $110/second child, and $105/each child after two.
